Semaglutide as a Reference GLP-1 Agonist: Trial Record, Receptor Biology and Structure

October 30, 2026 · 3 min read · LiliPeptide Authorized Store research desk

If a metabolic study needs a reference compound whose literature can be checked line by line, semaglutide is the obvious candidate. Very few synthesized peptides have been examined as thoroughly, and the published record behind it is far larger than what exists for a typical research peptide. This page starts with that record, then works back to the receptor and finally to the molecule itself.

Start with the trials: SUSTAIN, STEP and SELECT

Human data come mainly from two landmark programs. Type 2 diabetes was the subject of SUSTAIN; obesity was the subject of STEP. Over 68 weeks in STEP 1, people on semaglutide lost about 15% of body weight on average, compared with roughly 2.4% for those given placebo. A later published trial, SELECT, found cardiovascular outcome benefits in people with obesity who did not have diabetes. Those findings are the main reason GLP-1 biology leads metabolic research in activity today.

Questions labs are working on now

What happens at the receptor

Semaglutide acts on one target only, the GLP-1 receptor, which is a class-B GPCR. That receptor sits on pancreatic beta cells, on neurons in the hypothalamus and hindbrain, and in other tissues. Once it is activated, cAMP rises; insulin release is boosted, though only in a glucose-dependent way; the stomach empties more slowly; and appetite circuits in the brain respond. Because glucagon and GIP receptors are left untouched, researchers comparing the newer multi-receptor compounds use semaglutide as the single-receptor reference (see the tirzepatide and retatrutide comparison).

How native GLP-1 was re-engineered

The natural hormone survives only minutes in circulation. Semaglutide, a 31-amino-acid analog of human glucagon-like peptide-1, stretches that to about a week through three deliberate changes. Position 8 carries an Aib substitution, so DPP-4 can no longer break the chain down. A C18 fatty-diacid lets the molecule bind albumin, and a spacer tunes that binding. The week-long half-life is what made a once-weekly schedule possible in the clinical programs.

Handling notes

Research material is supplied as a lyophilized powder. Mix it gently with bacteriostatic water, then keep the solution cold and dark. The fatty-acid chain makes semaglutide more surface-active than small peptides, so agitation produces foam, and foam is a genuine route to degradation. When you check identity on a certificate of analysis, the mass-spec value to look for is around 4114 Da.

Semaglutide at LiliPeptide

The store lists Semaglutide in 5mg, 10mg, 15mg, 20mg and 30mg, and a Cagrilintide + Semaglutide Blend in 5mg and 10mg.

Our COAs page has no Freedom Diagnostics Testing report for semaglutide or for the blend. The closest entry is a report on a Cagrilintide 10mg sample (batch LIL-011826-RD, tested 21 Sep 2026, report LILI2609180941). It covers only that cagrilintide sample and tells you nothing about semaglutide or the blend. Cagrilintide on its own is listed as Cagrilintide (5mg, 10mg).
